Mask of choice.

New image and use element frame 2 edit copy and paste as a new layer on your new image. Select inside the middle and choose a paper of choice invert and delete. Move under frame.

Choose another paper of choice place behind your frame. Layers new mask from image and merge together your mask and paper.

Move your tube to the middle and move it under frame. Duplicate it and bring that one to the top. Erase the bits hanging over the frame.

Element 62 lower left hand and place it on the frame.

Elements 17, 18 and 19 arrange in the lower left below frame.

Element 28 move next to the lips. 

Element 26 arrange in the middle.

Element 24 lower right and open 4 arrange it next to element 24.

Rose element 23 place it in the hand of the tube.

Element 59 rotate it 90 to the right all layers checked. Place in the upper right hand.

Last element 37 move it behind your frame. Duplicate image flip. 

Add name and copy right information.
